All visits are arranged in advance to protect the privacy, routine and wellbeing of the people we support.

Before you start

Please submit your request at least 48 hours in advance. Requests are reviewed during Fina business days only (Monday–Friday, excluding Queensland public holidays). Requests submitted on a weekend or public holiday are processed on the next business day. Submitting this form does not confirm your visit.

Fina clients depend on routine. Requested visits are subject to approval and availability, and approved visits will be strictly timed.

Please arrive 30 minutes before your approved visit time to complete the required onsite induction. Induction must be completed before every client visit.

A visit may be denied or cancelled on the day due to client circumstances, safety requirements or operational considerations.

To avoid delays, please submit your request with enough notice for it to be reviewed, and ensure you arrive on time for your induction.

Please provide only information relevant to the visit — do not include unnecessary personal, medical or confidential information unless specifically requested.
